Comparison of the Emotional Schema Therapy augmented Exposure and Response

Abstract :
Introduction: Obsessive-Compulsive Disorder (OCD) is defined in the Fifth Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) with annoying and unpleasant thoughts or images (Obsession) associated with repetitive and unwanted actions (Compulsion). The purpose of this study was to compare Emotional Schema Therapy augmented Exposure and Response Prevention )ESTERP) with ERP and Pharmacotherapy in improving the symptoms of OCD patients. Method: This study was a randomized clinical trial. The present study was all patients with OCD who referred to hospitals and clinics in Tehran. 45 patients with OCD were selected by purposeful sampling, 15 patients were randomly assigned to the ESTERP group, 15 to the ERP group, and 15 to the Pharmacotherapy group. After randomly replacing patients in three groups, all patients commpeletd the Yale-Brown Obsessive – Compulsive Scale Second Edition (Y-BOCS-II), Four Systems Anxiety Questionnaire (FSAQ), Beck Depression Inventory (BDI-II) before and at the end and 2 months after the treatment. Then, data analysis using Nonparametric tests and Reliable Change Index (RCI) were performed. Results: There was a significant difference between the treatment of ESTERP with ERP and Pharmacotherapy in the symptoms and severity of OCD, anxiety(except for depression variable) in patients with OCD. In addition, findings of clinical significance, ESTERP compared to the other two treatments showed more significant changes in symptoms and severity of OCD, depression (except for anxiety variable). Conclusion: Both ESTERP and ERP treatments are effective in patients with OCD and both lead to more improvement in patients symptoms than Pharmacotherapy.
